Student Checklist

• Part I of  the Application For Employment Certificate completed and signed by a parent
• Copy of document with Proof of Age (e.g. birth certificate, passport, etc.)
• An original version of a:

• New York State doctor's note (stating the student is "fit for employment")
• Certificate of physical fitness completed by a NY State physician

All (3) items should be brought to our Work Based Learning Coordinator
