The NYC School Survey for Families, Students and Teachers opens today!
Each year, we ask families, teachers and students in grades 6-12 to take the NYC School Survey. The survey helps school administrators understand what key members of their schools community think about the learning environment and the quality of education children are receiving. School leaders can use feedback from the survey to make the school a better place for learning.
Families can choose to take the survey on (1) paper or (2) online.
(1) Each paper survey will be packaged in an envelope with the survey and pre-paid return envelope enclosed. Each family will receive a survey in English unless their home language is any of the NYCDOE-supported languages.

www.nycschoolsurvey.org by entering their survey access codes which is also printed on the survey.
**The Family access code is "f" followed by their child's nine digit students identification (OSIS) number. Note that "f" must be lowercase.
**The Student Identification (OSIS) number can be found on their report card, student ID card, or NYCSA Account
